Social Prescribing
Social Prescribing is a way for individuals to be referred to a range of non-clinical services and community supports in their area or locality. This is done with the aim of improving ones physical and mental wellbeing.
About Social Prescribing
There are multiple factors that determine a person’s physical and mental health and wellbeing (Individual, Social, Community, Living, Working, Healthcare, Housing, Cultural and Environmental)
When these factors are altered in some way, it can have a negative effect on a person’s overall physical and mental wellbeing.
What does the program offer?
- The Social Prescribing Program offers a confidential, trusting, user friendly support, that will help you find out more about activities and groups that are going on in the area.
- It also offers a structure wellbeing plan that is tailored to your needs and what interests you.
- Anyone over 18 years of age can take part in the program.
- Anyone who feels they need some support for your physical and mental wellbeing
- Anyone who is feeling lonely, isolated, depressed or anxious
- Anyone who would like to get out more in their community and meet new people
- Anyone who would like to learn or improve a skill or a hobby.
- Anyone who feels like the service could benefit them in any way.
- I will receive a referral from a healthcare professional (GP, OT, PT) or a community support service (Brothers of Charity, SICAP, ALONE, National Learning Network etc..) regarding a participant who may be interested or eligible for the program. The referral will provide details of the participant and why they are being referred.
- I always ask the referrer to talk to the participant and get their consent before being referred onto the project.
- I will have an initial assessment with you to talk about your needs and wants and what you would like to do.
- I will then link you to services, courses, groups or classes that you would like to engage in. I can support you by contacting them or I can give you the independence and you can link in with them on your own.
- I will then check in with you to see how you are getting on in the activity that you have been linked to. After 3 months we will have a review and see if you are happy to continue by yourself or if you would like more support.
- You can contact the Co-ordinator directly.
- You can contact your healthcare provider (GP etc.) And ask them to refer you to the project.
- You can get a family member or a friend to send in a referral.

- Vita House (Self-help and personal development programs as well as counselling & psychotherapy)
- Family Resource Centre’s (Provide a grange of group activities and groups suitable for all)

- Roscommon Sports Partnership
